Brief Paperwork

We value your time. If you are considering a consultation with our office, please call to schedule your no-charge consultation. After scheduling an appointment, please complete our secure, protected intake forms online, before your appointment.

If you are not able to complete intake forms online please allow 30 minutes before your appointment to complete the necessary forms.


Consultation

Whether completed online or in person, once your required forms have been completed you will have a private consultation with the doctor to discuss your personal health concerns.


History and Examination

The doctor will ask you various questions related to your symptoms in order to determine what your actual problem is. If you do decide to continue with treatment, an exam and a number of specialized tests will be performed immediately following your consultation, to determine which tissues are affected and to which extent.


X-ray Studies

Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. If you have had x-rays in the past 6 months please let us know ahead of time.


Surface EMG & Spinal Thermography Scans

These scans are a non-invasive way of determining the extent of nervous system interference. Within a few minutes the doctor will be able to look at a "report card" of your nervous system function. The scans involve wearing a gown in our private exam room for a neck to waist evaluation with one of our trained staff members. Children are welcome to have parents accompany them into the exam room.


Care Plan Creation

Once finished with the examination and all appropriate studies, patients will generally schedule a follow-up appointment to view results. Our chiropractor takes a careful approach to patient care. All images, scans, and information from you will help to reveal potential underlying issues. Treatment is based on your actual needs, and your individualized plan takes careful attention and time to create. Plan on your first adjustment taking place on your second visit to our office.


Home Instructions

Most patients will be given instructions for certain activities or procedures to be conducted at home. This may include ice or heat application instructions as well as avoidance of certain activities or positions.


Schedule Your Next Appointment

You will be given an appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Patients generally receive their first adjustments on their first or second visit to our office. Treatment options for your condition will be discussed during the following visits.
